School-Based Health Center
The ETHS School-Based Health Center (SBHC) is a full-service health clinic located inside the school and operated in partnership with Endeavor Health. Separate from the Nurses’ Office, the SBHC provides a wide range of medical and mental health services to help students stay healthy and access care in a safe and convenient setting. The SBHC is staffed by Endeavor Health medical professionals, including pediatricians, nurses, and social workers.
How to Make an Appointment
To schedule an appointment at the ETHS School-Based Health Center, students or parents/guardians can:

Appointments are encouraged, but walk-ins are welcome based on availability.
Who Can Use the Health Center?
All ETHS students are eligible to receive services at the School-Based Health Center. A signed consent form from a parent/guardian is required for students under age 18 before a student can receive most services.
- Students miss less class time because they don’t need to leave school for appointments.
- Care is private, easy to access, and specifically designed to meet the needs of teens.
- Services are delivered by medical and mental health professionals from Endeavor Health—offering the same expert care you’d receive at a clinic or doctor’s office.
- Services are available to all ETHS students, regardless of insurance status or ability to pay. No student is ever turned away for financial reasons.
Services Provided
The SBHC offers confidential, comprehensive care to students that includes:
- Physical exams (including sports and school physicals)
- Immunizations and vaccinations
- Lab testing
- Diagnosis and treatment of common illnesses and injuries
- Sexual and reproductive health services (including STI testing and treatment)
- Reproductive care
- Chronic condition management (like asthma or diabetes)
- Health education and wellness support
- Mental health services (including individual therapy and crisis support)
- Acupuncture
How to Access Services
Students can visit the School-Based Health Center by appointment or walk-in, depending on availability. To make an appointment or get more information, contact the SBHC via email or phone. If it's your first time using the Health Center, a parent or guardian must complete a Consent for Services form, available online and in the Health Center.
Cost and Insurance Information
The School-Based Health Center provides care regardless of a student’s insurance status or ability to pay. No student will ever be turned away due to cost.

Note: Free immunizations are available only to students who qualify for the federal Vaccines for Children program.
Consent for Services
Parent/Guardian Consent for Minors (Under 18)
A parent or legal guardian must sign a consent form for students under 18 years old to receive services at the ETHS School-Based Health Center.
Note: The Consent by Minors Medical Procedures Act (410 ILCS 210 Section 4) is an Illinois state law that allows minors aged 12 and older to independently consent to certain medical services without parent/guardian approval. This includes care related to sexually transmitted infections (STIs), substance use disorders, and the effects of drug or alcohol abuse by a family member. The minor’s consent is considered legally binding, and healthcare providers are not obligated to inform the parent/guardian. This approach aims to support students in seeking necessary care without barriers that might keep them from accessing health services.
Student 18 and Older
Students who are 18 years or older can provide consent for all medical services at the ETHS Health Center on their own. Parent/guardian consent is not required.
Physical Exams
A physical exam is a routine health check-up performed by a healthcare professional to assess a person’s overall health and wellbeing. For students, physical exams are often required for specific activities, such as sports participation. The School-Based Health Center at ETHS offers physical exams for students, including:
- Physical Exams for Incoming 9th Graders
- Sports Physical Exams for 9th–12th Graders
- Routine Physical Exams: Annual, Camp, Work, and College

Cost and Insurance Information
The School-Based Health Center provides care regardless of a student’s insurance status or ability to pay. No student will ever be turned away due to cost.

A receipt will be provided at the time of payment. SBHC receipts can be used for submitting reimbursement requests to private insurance companies.
*A reduced fee is available for students who qualify for financial assistance.
Flu Vaccines
Students and staff members can receive their flu vaccines at the School-Based Health Center. The Centers for Disease Control and Prevention (CDC) recommends getting vaccinated in the fall to provide protection during the peak of flu season. Students Under 18
Students under the age of 18 must have a signed consent form from a parent or guardian to receive a flu shot at the SBHC.
Cost of Flu Vaccines
The cost of the flu vaccine depends on the student's insurance status. For students with insurance or no insurance, the cost can range from $3 to $17. The flu vaccine is available at no cost to students who have Medicaid.

Student Health Center Ambassadors
The Student Health Center Ambassadors are a group of ETHS students who serve as peer leaders to help promote health, wellness, and access to care among their classmates. Health Center Ambassadors work closely with SBHC staff to:
- Serve as the student voice for the SBHC
- Raise awareness about available services and resources
- Plan and implement school wide health initiatives around topics such as STI prevention, blood donation, and mental health
- Promote safe, informed, and inclusive health practices
