Nurses' Office
The Nurses' Office at Evanston Township High School provides a wide range of health services to support student wellness and safety during the school day so they can fully participate in learning and school life. Whether you’re not feeling well, managing a chronic condition, or need help with medications or health plans, we’re here to help.
No appointment needed, but a pass is required during class time. If you arrive during a passing period, you need a pass from your next block's teacher.

To help keep our school community healthy, please follow these guidelines about when to stay home or go home if you’re feeling sick.
Health Plans & Medication at School
If your student has a medical condition that requires a care plan, or if they need to take medication during the school day, please work with the Nurses' Office to set up the necessary paperwork. These plans provide staff with specific instructions on what to do in case of a medical emergency and ensure your student gets the care they need quickly and safely. All forms should be updated every school year and submitted to the Nurses’ Office before the first day of school—or as soon as your student is diagnosed with a new condition.

Required Medical Forms
State law requires that students submit a physical examination and documentation of the required immunizations to complete enrollment in Illinois schools. A physical and proof of immunizations is required to begin Grade 9, and for any transfer students. Students without these medical records will not be allowed to attend school (including Summer School) until requirements are met.
Turn in forms to the Nurses' Office (N121) or drop forms off at Entrance 1 or 2 in a sealed envelope labeled "Health Services." For questions, call the Nurses' Office at (847)424-7260.
Student and School Health Data
In accordance with state law (Public Act 097-0910), the ETHS Health Services Department collects student and school health-related data. This includes immunization data that schools are required to submit to the Illinois State Board of Education annually by November 15.
