Start by completing the New Student Registration Form online. Create a new account or log in using your existing credentials (if you are a parent/guardian of a current ETHS student or current D65 student).

Be prepared to upload the following required documents:
- Student’s Birth Certificate (with certified English translation, if applicable)
- Proof of Residency (see table below for acceptable documents; one document from Category A and two documents from Category B).
- Current Medical Records (Physical Exam, Immunization, and Dental Records. View more information and the full list of required forms at eths202.org/MedicalForms.)
Additional documents that may be necessary to submit:
- Current IEP/504, if applicable
- Official transcripts from all high schools attended
- Withdrawal Form from the last school attended -or- 8th Grade Diploma
- Illinois State Board of Education (ISBE) Student Transfer Form (if transferring from Illinois public school)
If you are unable to submit the above documents to the online enrollment form a lockbox is available for document drop off at Entrance 2. If you have questions about these documents, contact the Residency Officer at residency@eths202.org.
If your student is transferring from a private school or out of state school, at time of submitting enrollment documents proving residency a “Good Standing Form” will be faxed/emailed to the last school attended. Once the form is returned to the Registrar’s office and found that your student is leaving in “good standing”, you will move forward in the enrollment process.
Be sure to click “Submit” at the end of the online registration form. ETHS cannot access your form until it’s submitted.
Enrollment is not complete until required documents are received and approved. Students will not be able to attend ETHS if residency cannot be verified or if required medical documentation is not submitted. After you complete the enrollment form, you will receive an email confirming whether your enrollment is complete or if additional documentation is needed.
