Incoming Freshmen Registration

Start by completing the New Student Registration Form online. Create a new account or log in using your existing credentials (if you are a parent/guardian of a current ETHS student or current D65 student).

Be prepared to upload the following required documents:

• Student’s Birth Certificate (with certified English translation, if applicable)
• Proof of Residency (see table below for acceptable documents; one document from Category A and two documents from Category B). 
• Current Medical Records (See the Incoming Freshmen page for more information) 

Be sure to click “Submit” at the end of the online registration form. ETHS cannot access your form until it’s submitted.

Enrollment is not complete until required documents are received and approved. Students will not be able to attend ETHS if residency cannot be verified or if required medical documentation is not submitted. After you complete the enrollment form, you will receive an email confirming whether your enrollment is complete or if additional documentation is needed.

Required Residency Documents

ETHS use placement tests as part of the course placement process for incoming freshmen. Placement testing is used to help determine appropriate course levels for your student.

The next step in the enrollment process is to request courses for your student’s freshman year. Students will submit their course requests in person at Course Request Night, held in January. Registration must be completed before course requests can be submitted.

During Course Request Night, incoming freshmen and their families will learn about graduation requirements, elective options, and summer school opportunities. Students will enter their course requests online during the event. 

To prepare, families are encouraged to review the online Course Request Guide and complete the Course Request Worksheet in advance. Discussing course options as a family ahead of time can help students make informed decisions about their freshman year schedule.

While ETHS prioritizes student preferences when assigning courses, final schedules depend on placement results, availability, and other factors.

Student fees are due annually before the start of the school year. Fees can be paid online, by mail, or in person at the ETHS Business Office (Room H108). An itemized fee breakdown is available on the Pay Fees page.

Approved by the District 202 Board of Education, student fees help offset the cost of programs and services that are not fully funded by tax revenues. Like many public schools, ETHS asks families to pay fees to support valuable educational resources—such as textbooks, lab materials, and technology—that enhance student learning.

ETHS offers fee waivers for families who qualify for financial assistance. To apply, submit a Financial Assistance Form.

Freshman Orientation is the final step in the registration process and a key event to help students feel prepared and confident for the start of high school. All incoming freshmen are expected to attend orientation, which takes place in early August.

At Freshman Orientation, students will:

• Meet ETHS leaders and staff
• Receive their class schedule and locate classrooms
• Learn more about school culture and support resources
• Receive their Student ID and Chromebook

A Certificate of Child Health Examination Form (physical exam) is required to begin Grade 9. This physical exam is a routine health check-up performed by a healthcare professional to assess a student’s overall health and wellbeing. For students, physical exams are required for enrollment and specific activities, such as sports participation.

Incoming Grade 9 students must submit a physical exam dated within one year of the first day of school. Note: Students attending Summer School must submit their physical exam prior to the start of summer classes.

The Certificate of Child Health Examination Form is the only physical exam form that satisfies proper school immunization documentation requirements in accordance with state law.

State of Illinois law requires each student to show evidence of the immunizations outlined in the Physical Exam and Immunization Requirements document below. For detailed information, visit the Illinois Department of Public Health website.

Immunization Requirements

In compliance with Illinois Public Act 093-0946, by May 15th of that school year, students entering grade 9 are required to have a dental exam on file. School dental examinations must have been completed within 18 months prior to the May 15 deadline. Download this form and have it completed, signed and dated by your child's dentist.

Parents/guardians requesting a religious exemption must submit the Illinois Certificate of Religious Exemption form signed by parent and student’s healthcare provider. The form will be required to start classes.

Illinois school code requires schools to request an Asthma Action Plan for each student with asthma. The doctor, nurse practitioner or physician's assistant that manages your student's asthma can complete this form or use their own form. Once completed and signed by the parent or guardian, the form must be emailed to nurses@eths202.org.
