Health Forms for Students
This page provides all required health-related forms for ETHS students. It includes documents needed for the start of the school year, as well as forms for specific medical conditions, immunizations, physical exams, and prescribed medications.
Families should review the requirements, download the appropriate forms, complete them with their healthcare provider, and submit them to Nurses' Office by the stated deadlines. Keeping student health information up to date helps us meet state requirements and ensure a safe, supportive learning environment.
Back to School Forms
State law requires that students submit a physical examination and documentation of the required immunizations to complete enrollment in Illinois schools. A physical and proof of immunizations is required to begin Grade 9, and for any transfer students. Students without these medical records will not be allowed to attend school (including Summer School) until requirements are met.
Turn in forms to the Nurses' Office (N121) or drop forms off at Entrance 1 or 2 in a sealed envelope labeled "Health Services." For questions, call the Nurses' Office at (847)424-7260.
Health Plans & Medication at School
If your student has a medical condition that requires a care plan, or if they need to take medication during the school day, please work with the Nurses' Office to set up the necessary paperwork. These plans provide staff with specific instructions on what to do in case of a medical emergency and ensure your student gets the care they need quickly and safely. All forms should be updated every school year and submitted to the Nurses’ Office before the first day of school—or as soon as your student is diagnosed with a new condition.
