Transition Education and Mindfulness for Success
The TEAMS Transition House at Evanston Township High School is a specialized program designed to support students with disabilities as they prepare for life after high school by providing individualized instruction and real-world experiences that help students build the skills necessary for independence, employment, and community involvement.
Program Overview
Transitioning from high school to adulthood can be challenging. The Transition House offers a safe, supportive environment where students develop practical skills, self-advocacy, and confidence needed to navigate adult life.
The TEAMS program serves adult learners between the ages of 18 and 21 who are preparing for post-high school life. We aim to support students in achieving the greatest level of independence possible by providing personalized instruction, practical skill-building opportunities, and meaningful community experiences tailored to each student’s unique strengths and goals.
Program Components
The TEAMS program operates through a collaborative service model involving students, families, school staff, and community agencies who work together to develop and implement individualized transition plans that meet each student’s goals.
